The Systems Health Framework is SBD's proprietary model for evaluating and improving facility operations. It examines five domains that every operation depends on — how work enters the system (Intake), how it gets done (Execution), whether leadership can see what is happening (Visibility), who owns performance and how it is enforced (Governance), and whether the operation gets better over time (Continuous Improvement). If any domain is weak, the entire system degrades.

You complete a 25-question assessment online. It takes about 10 minutes. The questions cover all five domains of the Systems Health Framework — how work enters your system, how it gets executed, whether leadership has visibility, how accountability is enforced, and whether the operation improves over time. Your scores are calculated automatically and a full report is generated based on your responses.

Yes. The online self-assessment is free and available at any time. There is no sales call required and no obligation. If you want SBD.Services to walk you through the findings, interpret the results in context, or help design an implementation plan, that is a separate engagement.

The self-assessment is completed online. If you engage SBD.Services for a deeper evaluation or implementation support, that can be conducted on-site, remotely, or a combination depending on the scope and your facility's needs.

You receive your report immediately. Based on your Systems Health Score, we recommend a next phase — either a 30-Day Stabilization Plan for reactive operations, an Implementation Sprint for facilities with emerging structure, or Ongoing Governance Advisory for operations that are structured but need optimization. You decide whether to move forward. There is no pressure and no automated follow-up sequence.
